Blackburn with Darwen

The Community Matron and Pulmonary Rehabilitation Team in Blackburn with Darwen provide an integrated Community Respiratory Service (CRS). This service is available for patients undergoing acute exacerbation or deterioration of their Chronic Obstructive Pulmonary Disease (COPD) where patients are at risk of hospital admission.

The Community Matrons and Pulmonary Rehabilitation staff are senior clinicians and highly experienced in the treatment and management of Chronic Obstructive Pulmonary Disease (COPD). They work together alongside your GP and yourself to manage and treat an acute exacerbation and deterioration where there is an immediate risk of a hospital admission.

Our aim is to provide coordinated personalised care and support you, your family and your carers so that you can stay safe and healthy at home and manage your condition where possible in the community.

The Community Matrons and Rehabilitation staff will support you to recognise the early warning signs and symptoms that your condition is deteriorating and act quickly to prevent admission to hospital and provide ongoing support and management through your recovery.

How to access this service

Patients who can be referred:

  • Patients who are registered with a Blackburn with Darwen GP practice, who already have a diagnosis of COPD confirmed by spirometry
  • Patients experiencing an acute exacerbation or deterioration in their COPD
  • Self-referral by patients previously seen and known to the Community Respiratory Service

The service operates Monday to Friday 8am to 6pm.

Community Matrons act as a single point of contact for care, alongside providing support and advice. Your Community Matron will be your key co-ordinator between your doctors, the hospital and voluntary agencies. In addition, we will make sure your views are listened to and that your care is designed around your needs.

The Community Matrons in Blackburn with Darwen also provide a Community Respiratory Service (CRS) for new patients with a Coronary Obstructive Pulmonary Disease (COPD) diagnosis, who are acutely unwell and are having unscheduled admissions and A & E attendances. CRS provides treatment intervention and optimisation of treatment, following resolve of the exacerbation, with a view to increasing their self-care/self-management. Central Lancashire

The Cardio Respiratory Service are a team of experienced specialist nurses and physiotherapists who offer a wide variety of services for patients with Chronic Obstructive Pulmonary Disease (COPD) living in the Preston, Longridge, Chorley and South Ribble areas.

We offer personalised care and provide advice and education to help patients to manage their own condition, recognise the early signs of an infection and know when to seek medical advice. We aim to support people with COPD to stay healthy and remain in their own home wherever possible.

  • Spirometry Diagnostic Clinic for COPD
  • Admission Avoidance
  • Supported Hospital Discharge
  • Specialist COPD review (nurse or physio)
  • Breathlessness Management
  • Pulmonary Rehabilitation
  • Palliative Care

How to access this service

They can be referred by GPs, medical & nursing staff from both Chorley and Preston hospitals, Community Matrons, District Nurses and self-referrals from patients already known to us.

Where this service is provided

We see people at home or in out-patient clinics as appropriate.
