Community Matron Service

The Community Matron Service provides care in the community to meet the health needs of individuals/families within the localities of Preston, Chorley & South Ribble and Blackburn with Darwen.

The aims of the service are to:

  • Assessment by clinical examination and diagnostics
  • Offer independent non-medical prescribing for patients
  • Support individuals to achieve and maintain optimum health and independence, avoiding unnecessary hospital admissions through discussion, teaching, advice and provision of appropriate clinical interventions
  • Provide a multi-skilled high standard of care which is evidence based and supported by reflective practice, clinical supervision and professional education
  • Work collaboratively with partners to ensure early intervention, continuity and consistent care and integrated intensive support in the least intrusive setting close to the patient's home
  • Actively identify patients who may benefit from case management
  • Educate patients / families / carers promoting effective self-management
  • Prevent unnecessary GP and ambulance call outs and hospital admissions
  • Apply advanced clinical and leadership skills to ensure that the most appropriate care and support are delivered at all times
  • Ensure effective medicines management
  • Prolong and extend the quality of life of residents of Preston, Chorley & South Ribble and Blackburn with Darwen
  • Work collaboratively with all colleagues throughout the health economy and with the support of the patient’s Lead Consultant

 

Community Respiratory Service

The Community Matrons in Blackburn with Darwen also provide a Community Respiratory Service (CRS) for new patients with a Coronary Obstructive Pulmonary Disease (COPD) diagnosis, who are acutely unwell and are having unscheduled admissions and A & E attendances. CRS provides treatment intervention and optimisation of treatment, following resolve of the exacerbation, with a view to increasing their self-care/self-management. These patients can also self-refer back into the CRS service if they are having a further COPD exacerbation.

Referral is by Health Professionals.

The Preston Locality Community Matrons can be accessed Monday - Friday 08.30 – 16.30 for patients and their carers to contact for advice, be triaged and assessed.
