Forensic In-Reach Team (FIT)
We are the Forensic In-Reach Team, and have a number of work streams.
Personality Disorder assessment and liaison involves the diagnosis of personality disorder and subsequent gatekeeping of admissions to secure beds with regular reviews of people once they are in them, with a view to eventual community move
Autistic Spectrum Disorder service has similar aims to the Personality Disorder service. This is a new service and not yet fully operational
Prison Transfer service works with prisoners who are admitted to secure hospitals under the Mental Health Act, trying to provide some continuity and involvement along their pathway to ensure people are within the appropriate environment. This service also provides Prison Complex Case Forums in each prison across Lancashire on a monthly basis
In-patient support involves supporting in-patient teams in managing service users whose presentation in the acute setting put them at risk of being admitted to secure services, therefore try and prevent this
Forensic Community Clinics to community teams who are finding it difficult to manage the risks of violence, not specifically self-harm, posed by a service user
Mentally Disordered Offenders Clinical Lead provides a dedicated service across the county, including policy development, monitoring and support to locality groups, and act as a Multi-Agency Public Protection Arrangements (MAPPA) single point of contact to the Accountable Agencies
How to access the service
We can be accessed via the Secure Services care pathway at Guild Lodge. We do not accept referrals from the general public.
Where the service is provided
We provide the service to all in-patient and community services that come under step 4/5 services within Lancashire Care NHS Foundation Trust.
