Intermediate Support Team

The Intermediate Support Team(IST) is an integral part of the adult secondary care mental health service, with a  key focus on older adults with a severe and enduring mental health need, functioning as a multidisciplinary team, that focus on hospital prevention and facilitating timely discharge from hospital to the clients home or other place of residence. IST offers intensive short term support.

Effective, timely intervention in older adults with mental illness improves quality of life for themselves and their carers; it promotes independence, social inclusion, reducing morbidity, improves the outcome in physical ill health and the need for emergency hospital admission. (ref CSED).
 
Providing increased support available out of hours for patients means  more patients can be cared for in the community as opposed to inpatient setting.  Benefiting patients and carers by avoiding unnecessary admission, loss of independence and isolation from family / friends

We currently provide this service in the following areas;-

Blackpool, Fylde & Wyre         East Lancashire    
Lancaster & Morecambe
Preston, Chorley, South Ribble & Ormskirk

The Intermediate Support Teams provide short term (up to 8 weeks) intensive, multidisciplinary interventions to older adults who may have complex needs because of an organic mental illness (dementia) or because of a significant functional mental illness (such as depression). The service we offer is based on needs rather than age, so we do also see some younger adults. We offer a fast response to give short-term intensive assessment and support.

The main aims are:

  • Prevention of admissions to hospital
  • Facilitation of early discharge from hospital
  • Prevention of crises in the community
  • Prevention of admission to a care home and/or prevention of unnecessary moves between care homes

Blackpool Fylde & Wyre Intermediate Support Team will provide a seven day per week service, from 8 am to 8pm, including Gatekeeping Assessments, providing also Intermediate Care/Home Treatment interventions for cases on the IST caseload.

The Intermediate Support Team provides a service for Preston, Chorley,  South Ribble and Ormskirk.

Referrals will be triaged by the Single Point of Access and sign posted to the most appropriate team within Lancashire Care Trust.
