DESMOND BME Link Worker Role

Our Link Worker Mumtaz Youssef is available to interact and translate for people with diabetes from the Asian communities in Diabetes clinics, Nurse-led clinics and in their own homes and other suitable community settings.

The role ensures that the level of information and understanding allows the person to make informed choices and give informed consent where appropriate, supporting patients in a variety of community settings according to local health needs with regards to diabetes and structured education. Our Link Worker works alongside the Registered Professional Educator, supporting adults with Type 2 diabetes to take responsibility for their health, well-being and future lifestyle through the delivery of the DESMOND Patient Programme. The role utilises all available resources taking into account the lifestyle, gender and cultural background, which assists patients in planning and setting of achievable and measurable health goals.

Our BME Link Worker facilitates communication between the diabetes team and Asian communities, to overcome cultural barriers in various community venues or in the person’s home, engaging in effective communication with patients, relatives and carers to obtain and provide relevant health information translating when necessary for Gujarati, Urdu and Hindi speaking patients during consultations.

Our Link Worker supports the facilitation of Asian people to access the diabetes services. Other duties include various promotion activities for the diabetes education programme within the Asian communities, including community centres and religious organisations, provision of clinical information in the DESMOND Structured Education programme translating in Gujarati and Urdu when required.  In addition, uses translation skills and cultural knowledge in the production of information leaflets and care plans for the Asian community.
