    Home View, an innovative partnership between Lancashire Care Foundation Trust and Home Group that supports people with mental health problems.

    Homeview is a 24 hours a day, supported housing service providing short-term accommodation for adults aged between 18-65 with mental health problems who have recently been discharged. The service supports those who no longer need clinical care in an inpatient setting but are unable to return to their home.

    Steve Lewis, North Lancashire Service Manager at Lancashire Care NHS Foundation Trust said:

    “Our number one priority is to ensure that our service users are cared for in the most appropriate environment and by working with Stonham Home Group we are ensuring that the people have a safe place to go when they are discharged from hospital. 

    “Homeview is a unique service which is proving to be a great success in supporting people at supporting people in the area.”

    Rachael Byrne, Executive Director of New Models of Care at Home Group, said: 

    “Patients are often ready for discharge from hospital, but not quite ready to go home, and this service helps to bridge that gap. 

    “Our work with Lancashire Care Foundation Trust has responded to current needs in healthcare, and provided an invaluable service in helping patients rehabilitate.”

    The step down service contains 12 self-contained flats allowing people to enjoy privacy for an average of 6 weeks and a maximum of 13 weeks. The service is available to those who are registered with a GP in the Blackpool area and who have an accommodation or support needs. The service aims to improves the care pathway and choices for individuals following a hospital discharge by providing the right care, at the right time by supporting them to access suitable accommodation and reintegrate back to independent living.
