Contract Win for Lancashire Care

Lancashire Care NHS Foundation Trust has been awarded the contract to provide Type 2 Diabetes Education Services across Blackpool, Fylde and Wyre and Lancaster and Morecambe following a competitive tender process.

The service will be following the ‘Diabetes Education and Self-Management for Ongoing and Newly Diagnosed’ programme, otherwise known as ‘DESMOND’. This is the collaborative name for a number of patient self-management education modules for people who are newly diagnosed or have been living with Type 2 Diabetes for a long period of time. Modules include education for people who have been newly diagnosed with Type 2 Diabetes and a foundation programme for those who have been living with diabetes for more than a year.

The delivery of the service will commence in October 2015 and means that the Trust will now be providing this service across a large area of Lancashire.

Amanda Housley, Service Integration Manager at Lancashire Care NHS Foundation Trust said:

"We currently provide this service in Chorley, South Ribble and Greater Preston and are delighted to have been given the opportunity to deliver the DESMOND programme in North Lancashire and Blackpool, Fylde and Wyre. DESMOND supports lifelong education and promotion of self- management skills, and will improve the health and wellbeing of people living with Diabetes in these localities."

The education modules take place over a full day or two half day sessions, they are delivered to groups of up to 10 people with Type 2 Diabetes and attendees can bring along a friend or partner. For more information about DESMOND or to make a referral please contact the DESMOND Team on 01772 777620
