An education programme for people with Type 2 diabetes in Central Lancashire has received a prestigious national ‘Team of the Year’ Award 2018 from the Annual Awards Programme.
Lancashire Care NHS Foundation Trust, which delivers the Type 2 diabetes courses in Preston, Chorley and South Ribble locality, DESMOND (Diabetes Education and Self-Management for Ongoing and Newly Diagnosed), has been presented with the ‘Team of the Year’ award at the Annual Awards 2018.
The judging panel at the Annual Awards thought that the DESMOND team entry showed ‘star quality’ and the panel was impressed by the teams philosophy and principles. The awards ceremony was introduced in 2012 to reward the excellence in the DESMOND community and to highlight, champion and celebrate good practice.
Heather Platt, DESMOND Structured Education Referral Co-ordinator at Lancashire Care NHS Foundation Trust said:
“We are delighted to be recognised for doing an excellent job as a team and to be part of a service that supports good quality care”.
“Trying to manage a long-term condition is a difficult thing to do, we recognise that challenge and we support our participants to learn how to self – manage their health and learn to live well with Type 2 diabetes. Those small changes can make a big difference”.
Bernie Stribling, National Director of the DESMOND Programme, said:
“The standard was high with a degree of excellence demonstrated by many of the entries. However, the Lancashire team showed ‘star quality’. “It was clear that DESMOND is making a difference to the lives of people with, and at risk of, Type 2 diabetes in Lancashire, with the team going above and beyond in delivering the DESMOND Programme.”
DESMOND sessions are run from the Minerva Health Centre in Preston with additional locations across the Chorley and South Ribble locality.
